Q: can used on car 12 volt

Yes and no. The switch part will function as a switch but the 12vdc does not pack enough punch to light a bulb requiring 120vac. Try something that was made for it instead. Do a search on "Illuminated Rocker Switches 12 V". I found plenty of results.

Q: Is the light inside the switch an LED or incandescent?

It's a neon bulb. These tiny bulbs require at least around 50 volts AC to light, at around 2 mA. You need 120v AC to power it.

Q: does the light stay on all the time?

no. Only in the on position. the great thing about these switches , they are easy to replace. Remember not to mix up the 3 wires on the bottom. mark their position as removed.

Q: the switch I have is for water heater on rv will this switch work

Not sure of the exact amperage rating but think it will be too "light" amperage. Check your amperage requirements and compare to the specs.
